Lifestyle (change) programme

Lifestyle is a major determining factor in whether and when you end up suffering from high blood pressure, high blood sugar, high cholesterol or similar conditions, how many medications you have to take, and how much they limit you. The new coaching programme from santé24 helps participants achieve lasting, healthy changes in their lifestyle and invest in their quality of life.

Coaching for a healthy lifestyle

Type 2 diabetes, high blood pressure, lipid metabolism disorders, cardiovascular disease and musculoskeletal conditions develop slowly and often in combination. They can frequently go unnoticed and untreated because it takes a long time for them to give rise to specific symptoms. But even before they're detected, they can cause great damage to the body. In addition to genetic predisposition, lifestyle plays an important role in whether and when a person develops a particular condition. A sustainable, healthy lifestyle can go a long way towards delaying or even preventing the onset of disease, avoiding secondary problems and improving quality of life.

The santé24 lifestyle (change) programme is aimed at people who suffer from high blood pressure, type 2 diabetes or a lipid metabolism disorder, or those who have a predisposition (family/genetic or personal risk factors) to these conditions. Because overweight or obese people are at a higher risk of developing these diseases, they can also register for the programme.

What does the programme involve?

The one-year coaching programme provides services related to nutrition, exercise and mental wellbeing. At the start, you and your principal coach determine what actions in these areas are sensible, necessary and realistic and agree what consultations make sense for you individually and when you should have them. You set overarching goals that are reviewed and, if necessary, adjusted in seven recall sessions over the course of the coaching programme. You also set specific goals in the individual areas; these too are regularly reviewed and adjusted in the course of the consultations.
For people who have not yet been diagnosed with high blood pressure, lipid metabolism disorders, type 2 diabetes or being overweight, but who suspect that they may also be affected now or in the future, santé24 will initiate appropriate screening. Participants are registered for a blood test at one of Medisyn's partner laboratories and can make an appointment themselves online.

Telemedicine specialists guide them through measuring their blood pressure. Risk factors are also recorded. The lab values and blood pressure measurements, together with the risk assessment, will be discussed in each case with a santé24 doctor during a consultation regarding general internal medicine.
If any of the measured values are elevated, medical treatment and medication can be initiated during the santé24 telemedicine consultation regarding general internal medicine. Patients receive regular telephone support and guidance from santé24 doctors. They subsequently receive regular medical support online. In addition, a physical consultation to review the treatment is arranged once a year at a GP practice or the relevant specialist practices.

If desired, coaching for a healthier lifestyle can start at the same time; this is because at the beginning of the diagnosis you can do a lot to maintain your quality of life in the long term. If no atypical values are measured, participants receive a recommendation as to when a new check-up would be useful and, if necessary, are offered the chance to take part in coaching for a healthier lifestyle.

Programme exclusively for SWICA customers

The lifestyle (change) programme is available only to SWICA customers. Coaching is free of charge, but there may be costs for laboratory tests or psychological/psychiatric counselling. Participants will be informed in detail at the start of the programme about any costs. The programme is offered in English and German. You must be at least 18 years old to participate.
