As a customer with a FAVORIT MEDPHARM, MULTICHOICE or TELMED basic insurance plan, you can, in exceptional circumstances, fulfil your notification requirement by notifying santé24 of a doctor's appointment that has already been arranged or taken place using the form below. - In emergencies
- Appointments with an eye doctor, gynaecologist or paediatrician from the MEDPHARM directory
- In connection with treatment following a consultation by a SWICA partner pharmacy or santé24 if administered by a provider from the SWICA directories
- When getting treatment during a temporary stay abroad
FAVORIT MULTICHOICE
- In emergencies
- Appointments with an eye doctor, gynaecologist or paediatrician from the MULTICHOICE directory
- In connection with treatment following a consultation by a SWICA partner pharmacy or santé24 if administered by a provider from the SWICA directories
- When getting treatment during a temporary stay abroad
FAVORIT TELMED
- In emergencies
- Children up to the age of 12 can be treated by a paediatrician without first consulting santé24
- Appointments with an eye doctor or a gynaecologist
- Before follow-up treatment after consultations already agreed upon with santé24
